I do not know if there are any out there now, but I am planning on developing one in the near future. I just need things to slow down at work first so I can find the time. I created one previously for the desktop, and I definately see the need for a hand held version.

Check out http://www.eyehit.com/ From what I understand is they are in the works on creating such a thing using a barcode system.
I just recently got a copy and though I haven't had time to go through everything I can see this as being a very good program.

I just use the "Documents To Go" version of MS-Excel. What's easier than a spreadsheet? Make a template, its FULLY customizable, and its tailored to how YOU want it. Why bother with anything else?
