Egg incubation discussion

Hi all:
I have been having discussions about different incubation techniques. I know that some of us have trouble with hatching; and babies going full term and never coming out. (I hate it when this happens) Anyways; so I wanted to see what other people do. I dont know if it is your big secret or not.. but maybe we can sort it out to help us all.
Here is what I do:
1:1 water to perilite by weight. cup has no wholes. I do air exchange 1 time a week. 86-88 degrees. Cup of water in the incubator. During the later part of the period I open wholes in the container, 2 on each side.
