In my turtles' spring/fall enclosure (i.e. not outdoors), I just use those Habba-Logs or whatever they're called (the little half-trees). I set them down into the substrate so the turtles have to dig a little to get under them. I think they like the perceived security of feeling the top of the inside of the log against their carapace (makes them feel like they've wedged themselves in tightly).

I didn't make hide boxes but for indoor use I just bought a 3 gallon sized Rubbermaid storage container, cut a hole in it for a door & poof I have a hide box. They aren't pretty but they're functional.
If you want something more natural looking there are tons of mail order catalogs that sell them.
