Squirt, my RES, has health prbs

Squirt's been keeping eyes closed while basking, swimming.. no matter if light is on or off.
Isn't eating much.
Eyes look puffy and swollen but there's no discharge..
Should I take Squirt to the vet???
Any idea what's wrong?
