What is This?

Any idea what this could be? it was found in Birmingham, Alabama, on a rock path in my front yard. My mom thinks it's a baby snake of some kind with a deformed head, I'm thinking it could possibly be some kind of bizarre worm. What do you guys think?

From your description, there's a good possibility it is a type of leech. There is a slimy worm-like critter that has a disc-shaped head. They grow to a length of maybe six inches or so.
If it is slimy, you can definitely rule out snake.
