what's wrong with my box turtle's shell?

For the last few weeks i've been seeing that along the front edge of her shell the upper brown layer is startign to peel back. This is making the layer below of tan/offwhite shell show. I know it's not just her shedding the shell pieces off because the piece being peeled back is much thicker than that. Also on the top rear portion of her shell (near the back end of the keel) several small orange white spots appeared. I thought it was something stuck to her shell but when i tried to peel it off with my nail i could feel a piece of shell start to lift up. In her tank she has a water dish she likes to take baths in several times a day but the water can only come up to about 1/2" up her shell.Is this shell rot or something else? What should i do to treat this?

Box Turtles poop and peep in their water. Bathing several times a day, needs clean water several times a day. Could be some kind of disease you see caused by dirty water
