I have a SICKLY Lesser Siren

I have a Lesser Siren that has been in captivity for about 4 years. I have been his care taker for the last year. He recently (1 week) has developed white spots on his skin that have quickly become raw areas of exposed flesh. I have been treating his water with an antibiotic for fish for the last 2 days. I also moved him to a different aquarium. He seems to be getting rapidly worse. Does anyone have any suggestions? I would appreciate any help and the siren would, too.
