He needs more than a water bowl, he needs water deep enough his shell would be under, but shallow enough to touch the floor and poke his head up to breathe.
I doubt he's captive bred, cos most of them are WC turtles;very few folks (incl me) breed these guys and my babies don't just go to anybody. Since he is most likely WC, his gut is probably chock full of roundworms. His poops would be full of them if you see it.
post some photos of the enclosure.
I hope he's hiding under the leaves, cos that's normal for them. He needs TONS of heat, like 85-95 degrees, and a cooler side as well.
