looks OK, not great but OK.
Probably 4-6 weeks old...tail looks like a birth defect to me.
1/4" crickets will be fine. I start mine on larger ones (1/2"- just pop off the back legs) he should start eating pretty quickly if conditions are correct, babies are pretty adaptable.
