Since you've had a lot of vet tests, then that has ruled out any urinary infections.

I would try two litterboxes. Some cats want to poop in one place and pee in a different spot. Also try "Cat Attract" litter and see if that helps.

You might want to go through the classic "retraining" which involves putting a cat into a small area and then gradually expanding the area as the cat becomes proficient at using the litterbox.
