Poor baby!

It sounds like you and the vets have really tried. Here are a few ideas - you didn't mention them, so if you've done these things, please forgive..

First, has your cat been tested for allergies? There are pet allergists available in some places and you might have one close by that your vet can refer you to.

Second, has your cat had a sinus cat scan? I don't know if one is available anywhere for cats, but I know that as a human I had one to see what was going on in my sinuses. I am wondering about some sort of foreign object.

If you have a veterinary college within a reasonable drive, it might be worth your while to set up a consulation. Usually these schools are on the cutting edge of treatment and can help where other vets can't. Also several colleges such as Tufts and Cornell have hot lines vets can use to get expert advise - this would cost you but might be worth it. Talk to your vet about this.
