It's possible that this is cancer, but only a vet can tell you for sure and answer the questions you have asked.
If surgery is recommended and you are willing to have it done, I highly recommend cryosurgery. This would involve freezing the lesions, instead of cutting, and would be much easier on your kitty. He would probably still need anesthesia, but it would be for a shorter length of time.
Sometimes, in a senior kitty, especially one with health issues, it's best to not treat aggressively. This is something you would have to discuss with your vet. It may even be best to not treat at all.
I'm so sorry you are going through this. It's extra-difficult to face a decision in senior kitties. I have one with probable intestinal cancer, but he has kidney issues, is anemic, and too thin to put through surgery. In his case, he is being kept very comfortable on steroids.
