14 yr old low appetite

I have a 14 yr old who has a low appetite. Of course I immediately thought, kidney failure. Vet ran complete blood work. Everything was in the normal range. 2 items were in the "high" normal range. 1 I don't remember, the other was the bilirubin (I think) as he mentioned that it would indicate jaundice, but Jeffrey had no yellowing and liver function was good. He did say something about autoimmune problem with his immune system attacking his hemoglobin, but he said he showed no signs of anemia (red blood count good). He said he had inflammation and steriods would be the treatment. Since Jeff is difficult to pill, he gave him a long term steriod shot.

Jeff's history: he and his twin suffer from mouth inflammation. His has never interfered with eating, and the vet said his gums were only slightly red. He has had chronic sinus problems until 2 years ago when they stopped. He has not had any vaccinations in a year. He does not go outside. He has lost 1 1/2 lbs over the past year. He has always been a petite eater only eating small amounts at a time.

my course of action has been to supplement as much as possible. Initially egg, milk, babyfood mix in syringe. Now tuna juiced, condensed milk. This seems to prime the pump and encourages him to eat a bit, but due to the lack of food on his stomach, if he does "pig out" it comes back up. Food stays down if small amounts given throughout the day. He acts like he wants to eat, but smells it and changes his mind.
