Honestly your best chances to find out what went wrong were to have a proper and thorough necropsy done. If the carcass was refridgerated or frozen, this may still be a possibility.

Without any more information, we can only guess. Is there a disease present in your collection that only presented itself only after a long incubation period? Was this an individual problem like a stroke? Cancer?
