It is better to test for worms before giving medication, as you don't want to give unnecessary drugs. Your description makes me think that your cat is sick, so your vet will probably do a CBC and a blood chemistry, and possibly a retroviral panel. The sooner you get this diagnosed the better it will be for the cat.
