The primary concern in her situation is the chronic renal failure, as this is allowing toxins to build up in her blood, poisoning her. If anything, she needs more fluids to flush out the toxins, or use the protocol I suggested to improve her condition and possibly improve some kidney function. Improving kidney function should be your first concern, as even a grade 3 heart murmur is not life-threatening, while impaired kidney function is life-threatening.
