Have you tried putting one extra litterbox somewhere? The kitten just might feel the box isn't clean enough. Or perhaps one of the other cats is occasionally "spooking" the kitten when she tries to use the box.

I would recommend trying isolation again - even to the point of getting a large dog cage and putting the kitten in that.

Talk to your vet about the possibility of using anti-anxiety medication - also you might check and see if there is an animal behaviorist s/he can recommend. There are animal behavior hotlines with Cornell University and Tufts University that your vet can use to help solve the problem.
