Exactly what I use:



1 sheet printer paper, folded in half



Top portion:

Species: (write species) ID Number: (If applicable) Gender:

Common Name: (Like Cal King) DOB: Description: (high white, albino, etc)

Nickname: (Like Bvrk!) Purchased: (name of store/person, along with phone number) Notes: (problem feeder, biter, etc)



And then I draw a line under that, and just list date and what happened on that day. Be it feeding, shedding, poop, vet visit, introducing breeders...



I just tape it to the front of the cage, or for my display cage, it sits on top of the screen.

Easiest method of keeping tabs on my animals that I've found!
