I do not think its about fat, it looks like a skin infection. That snake is not huge or fat. Its looks normal and healthy, other then the bumpy scales.
I would take it to a vet. It may be that simple.
My GUESS is, its caused by being slightly undermetabolized. that is, it cannot reach its full operating temps. That can impact the immune system. Or lack of temp choices, which can also impact the immune system.
THose two events can also effect the placement of fat bodies(abnormally) Anyway, have a vet look into that.
