Hi, for what its worth, vets have told me(and us) with varanids that petroleum based antibiotics do not work with reptiles(or is it just varanids) They do block the entry of more bacteria, but do nothing for the exsisting infection.
I would think that an infection that advanced needs to be looked at systematically. That is approached more then topically. Also the key with lizards(most reptiles) is to allow the build up of a strong immune system. Which means control dehydration and allow the ability for the animal to get HOT as it wants.
I am sure you already know this stuff, but incase not, I hope it helps. Of course, if your unsure of how to treat any infection, then a vet should be consulted. Best of luck
